NOTE: this entry form is only for the Pro-Am Charity event being held the Wednesday evening before the start of the Super Billiards Expo! You will need to be at the Expo Wednesday, March 12th at approximately 5:30 pm to participate in this event.
To register for the Pro-Am Charity event, please print out this form, fill in the information clearly, and mail it in to the address provided below along with your check or money order for the entrance fee. After we have received and processed your entry, you will receive a notification of your entrance into the event. Thanks, and good luck!
